How does the interaction between the HLI physicians and the client's PCP work?

  1. Log in to Human Longevity's secure portal and downloading their own files

  2. Request Human Longevity to fax their medical records to the PCP

  3. Request Human Longevity to initiate a secure digital transfer of their medical records to the PCP

If a client has an actionable result requiring a physician referral, how does that process work? What if the client is outside of California?

Human Longevity has an internal network of some of the top physicians in the country. The client's physician will make a referral, based on the criteria and needs as well as the client's preferences. Human Longevity's medical team will work to accommodate the client's needs.

  • Clients can travel to a specific medical facility for their consultation.

  • Clients can request a provider closer to their home.

  • Clients can request a provider within their insurance network.

What is the timing of the client renewal process/paperwork and how is this process managed?

Human Longevity renews on an annual basis.

90 days before renewal

  • Human Longevity contacts clients to review any outstanding care or treatment.

  • Marqi Health sends a reminder to both the employer and the executive to determine if the executive will be eligible for their company-sponsored benefit.

30 days before renewal

  • Marqi Health coordinates with Human Longevity and sends new paperwork to employer and copies to executive.

If the executive benefit itself is COBRA-eligible – what would the rates be for COBRA? COBRA charges monthly – so, would it be a full $25,000 for the year (plus a 2% administration charge by our COBRA vendor) which would equate to $2125.00 monthly?

The Human Longevity Executive Program does qualify as a COBRA-eligible benefit and would be subject to the traditional 2% admin fee established by your COBRA vendor. When an employee is terminated from group health coverage, they have the option of purchasing the Human Longevity Executive Program benefit from the insurer.

However, this would not be a monthly payment. Human Longevity's Executive Physical is paid upfront, so the executive would pay the COBRA vendor $25,000+ 2% admin fee before visiting the clinic.

COBRA allows the executive to make their Human Longevity Executive Physical benefit tax deductible.

The particulars of your health care continuation right under COBRA are explained at length in the plan document/summary plan description for the plan, program or arrangement with which the Plan is integrated
